I hereby confirm and declare that I am knowingly booking and going to receive a firming, toning, therapeutic, non-sexual massage of my bare breasts and I agree to this treatment being given in a professional manner.
I acknowledge, agree and confirm that by signing this agreement/consent form that I fully understand that the massage that I am booking is a therapeutic, non-sexual massage of my bare breasts that I am going to receive prior to the treatment being given and that this type of massage therapy treatment is being given for reasons of general health, well-being and wellness thus helping to reduce cellulite in the breasts to help give a firmer and more toned appearance.
I confirm that I have made contact with Simon my professional massage therapist masseur in the full knowledge that I will be booking and receiving a non-sexual, therapeutic, anit-cellulite massage of my bare breasts to be given professionally in accordance with my own personal wishes and I understand that the massage will be given with my prior permission and only if I feel totally comfortable and relaxed to feel the benefits.
I confirm that I have personally invited my professional male massage therapist/ masseur Simon to visit my home, hotel room or office workplace to give me a therapeutic massage of my bare breasts after reading through his website in full which is www.holisticbodymassage.com and that I may have to receive the massage treatment when my breasts are completely bare, uncovered, un-draped, exposed and topless.
I confirm that I have read through anti-cellulite massage section within Simon's main massage therapy website and I that am booking a therapeutic, non-sexual, firming, toning, therapeutic massage of my bare breasts to help reduce cellulite.
I confirm, accept and acknowledege that I have read this section within the main massage therapy website regarding firming, toning, therapeutic, anti-cellulite massage of the breasts and I that I now know what to expect with a professional breast massage therapy treatment.
I hereby give full permission and prior informed consent for my massage therapist/ masseur Simon to allow him to give me a therapeutic, non-sexual, firming, toning, massage of my bare breasts to help reduce cellulite and give a more firm and toned appearance.
I also give my full consent for a massage of my bare breasts to be given if I also book a full-body naturist massage.
I do NOT give my permission and consent for any form of sexual massage whch includes massage of my breasts and I understand that this style of massage will only be given over the undraped bare breasts when I am topless.
I understand that Simon will remain completely professional at all times during a therapeutic massage of my bare breasts and I fully accept that I will be receiving a non-sexual massage of my bare breasts to be given as an holistic therapy and therapeutic treatment.
I hereby sign this legal delcaration consent/ agreement form to confirm that I am booking a massage therapy treatment with Simon that will include the non-sexual massage of my bare breasts to be professionally given with my full permission, approval, knowledge and that I agree to the massage of my bare breasts with my informed consent.

I hereby promise and confirm that I NOT transgress any pre-arranged boundaries and that I will always respect the pre-arranged boundaries, limits and comfort zones of the aforementioned client who has knowingly booked me to give her a massage therapy treatment that will always remain within my client's personal, pre-agreed comfort zones, thus making her feel happy, comfortable, safe and secure at all times.
I promise, assure and personally guarantee that all my holistic massage therapies given will be non- sexual and always remain within the pre-agreed boundaries of my female clients.
I promise and state for the legal record that I will NOT go beyond the boundaries of my client during the course of the massage and that I will remain professional at all times having full respect for my massage client and I confirm that I will always respect wishes/requests of my massage client at all times in accordance with this legal, binding agreement/consent form and in accordance with the client and recipient of any of my massage therapy treatments/services.
I promise that I will always remain faithful and true to all the pre-arranged agreements and that the massage will always remain within the personal comfort zones / limits of my client, thus enabling her to fully relax, secure and safe in the knowledge that although my client is booking a firming, toning, therapeutic, anti-cellulite massage of the bare breasts, the massage will NEVER, EVER go byond her personal wishes, requests, desires or trangress her pre-arranged boundaries.
